5. Consent Documentation
We use TrustedForm, a service of ActiveProspect, Inc., to create a certificate of your consent at the time you submit a form on our site. This certificate records the timestamp, your IP address, the form URL, and a visual snapshot of the disclosure presented to you at submission. TrustedForm certificates are retained solely for TCPA compliance documentation and are never used for advertising purposes.
6. Analytics and Third-Party Widgets
Our site displays a cookie consent banner when you first visit. Analytics cookies are only set if you accept them. If you decline, Google Analytics 4 (GA4) does not load and no analytics data is collected during your session. You may change your preference at any time using the consent banner, which can be reopened from the footer.
When analytics cookies are accepted, we use GA4 to understand how visitors use our site. GA4 data is processed under a Data Processing Agreement with Google and is not sold. We also embed a review display widget from Trustindex (trustindex.io) to show verified Google reviews; this widget does not receive personally identifiable information about site visitors and is not gated by the consent banner.
You may also limit analytics tracking at any time by adjusting your browser's cookie settings or installing the Google Analytics Opt-out Browser Add-on.
7. Marketing Partners and Lead Sharing
Midtown Home Improvements is the contractor — we perform home improvement work directly. In most cases, the information you submit is used only by Midtown to fulfill your request.
However, when Midtown cannot fulfill a homeowner's request due to project scope, service area constraints, scheduling capacity, or other operational reasons, that inquiry may be designated as an unfulfilled service referral and shared with our authorized fulfillment partner, RemodelingAdvisor.com, LLC. RemodelingAdvisor specializes in connecting homeowners with qualified home improvement providers and may in turn use their provider network to fulfill your original request. Providers within that network are bound by applicable data-sharing and contact rules.
What we share: name, phone number, email address, property address, and service type requested. We do not share payment information, government identification numbers, or unrelated personal data.
If you prefer that your information not be shared with our fulfillment partner, contact us directly at 1-888-725-0170 before or immediately after submitting your form and we will remove your information from any pending referral. See our Marketing Partners page for additional detail.
